This is my theory for who this is:

I believe it is Menachem Mendel ADLER, born around 1860, married to Channa'le, and the father of the following sons:

Mordechai Markus ADLER

He was probably born around 1873,  married to Hanna GABEL, father to
Suzi married BARD and Ethel married ADLER.
     Lived in Lubaczow.

Dawid Shmuel ADLER

He was born around 1880. Married to Elka nee LEVI, father of Moses born ca      1922/1924,  Jehuda Zvi born ca 1931, Mindel born ca 1936.
      Lived in Lubaczow.

Zvi Cwi Hersch ADLER

He was born in 1886, a merchant, married to Scheindla nee KALTER, father        of a daughter born around 1920.
     Lived in Lancut.

Majer Meir ADLER

He was born ca 1892, married, a horseman.
      Lived in Katowice.

I have an additional theory, namely that the Yizkor list for Lubaczow made in the early fifties, may have contained names of persons who had not been murdered in the Holocaust, but had died before the war.

Thought:
From the fact that Mendel ADLER named his son born around 1884 Zvi Hirsh, it seems like his own father had died between 1880 to 1884.

In the 1929 Business Directory for Lubaczow, the following ADLER are listed
