35 years old. Born around 1898. Son of Lazar and Scheindla CZERNIAK.
Israel Czerniak was born and lived in Nahaczów. That place had, according to Where Once We Walked, 131 Jewish citizens before the Holocaust. Today it is called Nahachiv and is located around 26 km SW of Lubaczow,across the border in the Ukraine.

Checking Jewish Records Indexing - Poland I found that in the little town of Jaworow ( now Yavoriv, Ukraine) Luzar CZERNIAK and Chane CZERNIAK nee CZERNIAK from Nahazów ( now Nahachiv, Ukraine) had a daughter in 1882 they named Rosze Zlate. The distance between Yavoriv and Nahachiv is around 25 km.
As to the decoration:
I have found only two graves in this cemetery that have a Star of David as a decoration -
